Service Transformation and Redesign Projects

Camden & Islington NHS Foundation Trust Health Based Place of Safety

We led a successful 6 months design phase of this project, mobilised key resources and stakeholders, developed the business case and secured funding of £1.4 million from the Trust Board and NHS England for the construction of a new purpose-built facility to enable people in a mental health crisis to get timely care and assessment in a dignified and confidential environment

City & Hackney Dementia Service

The City & Hackney Dementia Service achieved several significant milestones in its mission to improve the lives of individuals and families affected by dementia. One such accomplishment was leading the charge for change and investment, spearheading the mobilisation and consultation process for the redesign of the City & Hackney Diagnostic Memory Clinic.

In addition to this, the service also developed new approaches that focused on promoting better integration, maximising resource utilisation, and eliminating duplication. This included successfully integrating the East London NHS Foundation Trust and City and Hackney Alzheimer’s Society, which allowed for the delivery of the new CH Dementia Care Model.

One of the most innovative aspects of the City & Hackney Dementia Service was its commitment to ensuring that every patient and their carers or families had a named care practitioner to support them throughout their disease progression, from diagnosis to death or when they moved out of the borough. This approach resulted in the creation of a better-resourced, specialist service that provides both diagnosis and post-diagnostic support to people affected by dementia and the professionals involved in their care.

Hope and Restoration t/as H&R Training Professionals-training & education programmes

In collaboration with H&R Training Professionals, we successfully secured funding and implemented their first three skills training and back-to-work programmes across North London. Our work with the management team allowed for the creation and implementation of programmes that would enable individuals to acquire the skills and knowledge necessary to secure employment opportunities.

Additionally, we provided support to H&R Training Professionals in gaining accreditation as a learning centre for the Open College Level 2 & 3 mentoring programmes. This accreditation was a significant achievement for the organisation, as it established them as a recognised learning centre for individuals seeking to acquire knowledge and skills in the field of mentoring. These accomplishments have allowed H&R Training Professionals to continue growing and thriving in their mission to support individuals seeking to enhance their skills and knowledge.

The Shane Project-Reaching Communities Funding for MS activities

Our team had the pleasure of working alongside The Shane Project’s Board of Trustees and project team, where we played a vital role in securing their first substantial three-year funding from the Big Lottery Reaching Communities. This funding was instrumental in helping the organisation expand its services and support to people affected by MS.

In addition to securing funding, we also provided guidance and support to the project team and members in developing and designing activities for individuals affected by MS. Our collaboration helped to ensure that these activities were tailored to meet the specific needs of the individuals and promote their overall well-being.

Overall, our efforts in supporting The Shane Project have had a profound impact on their ability to provide support and services to individuals affected by MS, which has helped to improve their quality of life.
